Fees:
We will help you determine your insurance coverage but this is your responsibility.
Copays and charges not covered by your insurance are due at the time of service. Since many insurances have large deductibles we can work with you to set up a payment plan.
No Show and Late Cancellation:
If you schedule your initial appointment and do not show up or cancel late, we will give you another appointment should you request it, though a second such failure to keep your initial appointment will cause your dismissal from our practice.
